Multi-Site Subscription Form to the: American Journal of Health Behavior^TM, Electronic (online)

Consortia and Institutions with multiple sites require a multi-site subscription. Multi-site subscriptions include organizations located in more than one city or which have multiple locations in the same city administered independently. The multi-site subscription fee is based on the existing institutional subscription rate.
